In the first programme, Chris and Xand travel to one of the most disease-ridden places on Earth to live with the Bayaka Pygmies in the Congo Basin. This densely forested region of equatorial Africa is renowned as the incubator for the most deadly viruses known to man, including Ebola and HIV. Over the course of a month, the brothers try to understand how the Bayaka have managed to thrive for tens of thousands of years in such an unforgiving environment. Finally, however, it is the simple need to eat that presents them with their greatest challenge…

In the second programme the twins travel through Asia to examine the region’s attitude to pain control. They start their journey at the Hindu festival of Thaipusam in Kuala Lumpur, the capital city of Malaysia. Here, religious devotees appear to have an ability to endure the pain of piercings through the face and body by seeming to enter a blissful trance. The twins decide to take part as a scientific experiment for which, as twins, they are the perfect control. Xand prepares in the manner of a devotee by undergoing a week of religious instruction and abstinence, and Chris continues with his stressful job in London. The results are interesting – and unexpected. They then travel to Nepal to explore the region’s ancient Tibetan medical system, which does not include any specific medicines to deal with pain. Finally, Chris and Xand visit India, where the Federation of Indian Rationalists Association attempt to show that it is possible to cope with the pain of being suspended in the air from hooks using only the power of the mind.

In the third programme, Chris and Xand visit Chukotka in the farthest northeast region of Russia. An icy peninsula, it is washed by the Bering Sea, a section of the Pacific Ocean, and part of the Arctic Ocean. Its vast geographical area of over 737,000 km² is home to a population of only around 55,000. Much of the mainly rural population still exists on subsistence reindeer herding, hunting, and fishing. Their diet consists almost entirely of sea mammals, which is mainly protein and fat. Yet mysteriously, the people of Chukotka report no incidence of heart disease, and their cholesterol levels are incredibly healthy. Could this change the way we think about the diet/health connection, and go against everything the twins have been taught about nutrition? Chris and Xand are once again immersed in the culture, including the challenge of both eating and hunting walrus and whale. They are struck by the respect the hunters have for their prey, and learn about a lot more than medicine during their time with the Chukotka people…

In the fourth programme, Chris and Xand live with the Asháninka, the indigenous people of the rainforests of Peru. The Asháninka have been a people under siege since the days of the Spanish conquistadors, and current threats include those from oil companies, drug traffickers, colonists, illegal lumberers and illegal road builders. Even ‘benevolent’ individuals and organisations such as missionaries and conservation groups are having a negative impact on this beleagured nation. During their time in the rainforest, the van Tullekens learn about the ways of Asháninka healing and Shamanistic beliefs, including the powers of ayahuasca.
